I was thinking along the same lines as P Pixie's feedback -- go to the dental office, ask the receptionist "sorry I forgot when my next appt's supposed to be ..". After you get the info then ask said receptionist "is [hygienist's name] available? because if so I'd like to ask her out".
If she's available -- then go ask.
